An examination of systems base thinking in relation to what it is, how it will be utilized by the APRN-DNP, and how systems thinking improves health care today as well as in the future.
Systems thinking emerged from the general systems theory. The assumptions were derived from Ludwig von Bertalanffy (1901 – 1972). It could further be contended that philosopher Herbert Spencer’s (1820 – 1903) sociological analysis of society being likened to the systems in the body is another approach of evaluation systems thinking. Essentially, when systems’ thinking is perceived from the nature paradigm it encompasses a study of ecosystems. Interactions within the system and outside are interpreted (Mugadza, 2015). .
Similarly, when organizations are evaluated through a systems thinking point of view the function of each part distinctly is assessed as well as its relationship to whole. Systems theorists/ scientists contend that the full understanding of a problem requires interpretation of parts in relation the whole. As with the human body when a disease condition is diagnosed at first the system dysfunction is evaluated in relation to how the entire body functions manifesting signs and symptoms (Smith, 2013).
With respect to how systems thinking will be utilized by the APRN-DNP, first it must be recognized that systems thinking is a contemporary development in healthcare delivery organizations. Scientists have recognized it as a paradigm shift. According to the World Health Organization, Alliance for Health Policy and Research Systems quoted by Adams (2011) there are common systems’/ thinking characterizes in health care. Major classifications are self-organized; constantly changing; tightly linked; governed by feedback; history dependant and resistant to change (Adam, 2011).
The above diagram represents how system thinking could be utilized within a healthcare organization by an advanced practice registered nurse (APRN). While self-organization; constant change; tightly linked associations; feedback interventions; historical dependence and change résistance are attributes of systems thinking in healthcare delivery processes across the world, alternatives are available (Adam, 2011). The advance practice registered nurses’ (APRN) role makes a huge difference in establishing dynamic systems thinking in a high performance healthcare industry (Peters, 2014).
For example, systems’ thinking forges this category of nurse to employ strategies that ensure efficient delivery of healthcare through improvement in patient care experiences and populations. The advanced practice nurse uses systems thinking to execute quality measurement and improvement activities within the organizations they function. This is achieved by addressing preventative care needs; planning and engaging in patient outreach programs along with conducting research projects adding value to existing knowledge (Peters, 2014).
Astute systems thinking has brought into manifestation information technology in main stream health care delivery. Advance practice registered nurses are in the fore front of this paradigm shift being articulate in training staff to become updated system thinkers in the transition from paper records to electronic health recording systems. In an effort to improve healthcare quality advanced practice registered nurses (APRN) as systems thinkers have been given the mandate to utilize information technology as meaningful use in the clinical decision making process towards their pursuit of executing evidence-based practices ( Peters, 2014).
In explaining how systems thinking improve health care today, it can be proven through the development of systems in itself. The above diagram is one example of how health care within the past 10 years has evolved into a huge system consisting of managed care with many providers. More importantly, in the twenty first century, systems’ thinking has realized closure of the gaps between persons in society who could afford quality care and those for whom it was inaccessible. Addressing the dysfunction in one part allowing more Americans to afford health insurance has improved access to quality health care for minorities in the society. As such, systems’ thinking in my opinion has a great future in healthcare delivery.
