My proposed scholarly initiative is reflective of a model that is designed to transform healthcare and to facilitate a comprehensive and high quality care and treatment plan for patients with HIV/AIDS, using the Chronic Care Model. This model reflects a need for change in the context of chronic disease and how patients’ quality of life may suffer if they do not have the education, knowledge, and resources in place to self-manage their condition effectively and consistently (Chu & Selwyn, 2011). Changes within the context of this model will reflect a need to support the growth and maturity of advanced practice nurses in different roles through experience and communication to improve patient outcomes (Dreher & Glasgow, 2011). Furthermore, technology-based solutions must be used to bridge gaps in knowledge and resource allocation for advanced practice nurses in order to work with these patients as efficiently as possible (Pasricha et.al, 2013). Finally, educating patients regarding self-care is another critical component of the practice environment that will improve the health of these patients over time (Schulman-Green et.al, 2012).

At the advanced practice level, it is necessary to employ proven knowledge and techniques in favor of improving patient outcomes, and this is reflected in optimizing core competencies that include 1) Interprofessional Collaboration for Improving Patient and Population Health Outcomes and 2) Advanced Nursing Practice (AACN, 2006). It is important to emphasize the value and contributions of advanced practice nurses in developing and implementing a chronic care model for HIV/AIDS patients to promote self-care under which communication among different collaborative partners is critical to the success of the model and its impact on patient quality of life. This is an important step towards improving outcomes for patients and for developing a strategy that will encompass new objectives to improve the health and wellbeing of persons with HIV/AIDS.
