My performance in the DNP role has been challenging in many ways, but I have discovered new characteristics about myself in the learning process that will enable me to grow and thrive as a leader in evidence-based practice. Establishing my role in the field as a leader and a mentor requires due diligence and enthusiasm to facilitate improved engagement among other nurses (Dreher & Glasgow, 2010). Exploring my leadership capabilities at the doctoral level is essential to the application of the Chronic Care Model for patients with HIV/AIDS; therefore, I must explore the options that are available to facilitate progress and to ensure that this patient population is served well, using examples from the model’s application to diabetes and other conditions to improve disease management (Stellefson, Dipnarine, & Stopka, 2013).

It is important to expand my role as a doctoral-level nurse to promote an effective application of the Chronic Care Model to HIV/AIDS through the promotion of collaborative care and treatment to improve patient health and wellbeing across the spectrum (Woltmann et.al, 2012). This opportunity offers a strong learning curve to ensure that patients receive the best possible care and treatment on a comprehensive basis that will facilitate desirable results for HIV/AIDS patients, using approaches that are grounded in mental, social, psychological, behavioral, and physical wellbeing (Woltmann et.al, 2012). At the same time, I must be able to recognize limitations for this model and how they might contribute to difficult challenges for patients and for doctoral-level nurses in applying proven strategies that will enhance the delivery and quality of care that patients receive (Kadu & Stolee, 2015). Under these circumstances, my role as a leader requires a strong commitment to excellence and an opportunity to examine the key perspectives that will influence decision-making and progress in improving the quality of life of this patient population on a comprehensive basis to include physical, mental, psychological, and emotional needs.
