The Arc of Monroe Foundation, 1000 Elmwood Ave., Rochester, NY 14620 |
Name and Information for Contact Person |
Goal/Mission of the Agency for the Given Population
The Arc Foundation funds initiatives not traditionally funded by Medicaid or other government agencies. Through innovative, popular and effective events, the Foundation raises money that funds programs designed to enable individuals with developmental disabilities to be active, contributing members of their community. The funding can also be provided directly to the families of patients if specific care or services cannot be provided by the organization.
|
Services Provided by the Agency
The organization offers habilitation, self-advocacy, recreational and health services. Funding is usually provided through the Arc On-line Auction and Blue Jeans After D’Arc. While involved in numerous initiates across the county, the Arc Foundation is best known for Dream It, Do It, which is a program that works to grant wishes for individuals with developmental disabilities and their families. |
Assessment Process to Qualify for Services
The only assessment process that exists for individuals to qualify for funding is whether or not Medicaid or other government agencies would not traditionally fund the services that were required. The Arc Foundation focuses on providing to those individuals that are in the greatest need first, but it does not want to turn anyone away if the government cannot traditionally provide funding to those people that need services. The process begins with the referral process whereby a family member, friend or medical professional can refer a patient to the agency to receive their services. |
Timeline for Securing Services
The funding is usually provided within several weeks of applying. However, there are cases where fundraising may limit the exact amount of funds that can be provided to patients and their families. Many of the other services that are provided, such as health care and recreational services can be provided immediately. It really depends on the type of service being applied for and what the needs of the patient are based on the amount of available human, physical and technological resources. |
