In modern science information system is synonymous with computer technology. In my organization Kaiser Permanente is the information system provider whereby medical records are stored, exchanged and transferred. This is a document oriented database program. In describing the selected application it must be understood that the purpose of this database is to accurately manage patients’ records. The application is known as electronic health records. This is an evolving application, which was set into motion through the Affordable Care Act. It consists of databases responsible for exchanging accurate data among care providers and within the organization itself. Database characteristically entails organized collection of data, which is designed according to a mandatory. In this case it is the medical records standards (Gunter & Terry, 2005).

Description of a simple table structure, including records and fields, that might be used in the database

The first database structure came into existence sometime during the 1960s and was called navigational Database Management Systems (DDMS). A simple table structure inclusive of record fields that might be used in this database could be explained through application of the rational model. This structure facilitates database content evolution without having to constantly rewrite of links and pointers. Precisely, the rationality emerges in the sequencing whereby entities reference others interpreted as one-to-many relationship. This is similar to a traditional hierarchical model, and many-to-many relationship as well as navigational model. As such, a relational model can collaboratively express both hierarchical and navigational models (Lightstone, Teorey & Nadeau, 2007).

Explanation of fields that might be used as keys and indexes.

Fields that might be used as keys are login and defining documents such as Mark, Lion and Kitty. Storing information in optional tables like first and last is another useful key. Data base indexes are structures, which facilitate speedy recovery of data. The relational has been advocated as an efficient database indexing program whereby a copy of selected data columns from a table as shown above that can be easily searched very efficiently. Usually the guide to identifying the index is a low level disk block address or direct link to the complete row of data from which it was copied (Lightstone et.al, 2007).

Description any limitations or constraints related to the data and how it is structured.

Again database experts advocate that database transactions have their limitations. For example, database transactions represent a work unit encapsulating a series of operations. They include reading an object, writing as well as acquiring lock. Each transaction has distinct boundaries related to the program code included in special transactions commands. If a provider tries to post information within the database without a code it would not enter or the system will reject the information. Consequently, structuring of data must be in alignment with the transaction code to be accepted (Beynon-Davies, 2004).

Discussion of standardization

Standardization of database language is very important towards successful transmission of data from one health care institution to another as well as provider. Actually it is the medium used to communicate data. Other standardization protocols refer to keys and indexing patterns. All standardization really means is consistency of data storage, retrieval and exchange. There could be national standards as well as local consistency mechanism. However, without adequate standardization no data base could function efficiently as a document oriented program. This makes the system inaccessible and irrelevant (Beynon-Davies, 2004).
