The chronic care model may be difficult to implement in some cases because there are significant challenges related to its overall development and the ability to manage HIV/AIDS in the appropriate manner. There must be a greater emphasis on the ability to overcome challenges and to recognize the value of improving patient health and wellbeing in a manner that is consistent with these principles. Most importantly, without a strong learning curve, this process will not be achieved and will fall short in its efforts to achieve effective outcomes for patients (Leykum et.al, 2011). In addition, it is likely that there will be additional challenges related to the development of new perspectives to improve health and to also recognize the value of achieving greater quality of care through institutional support for this model (Nundy et.al, 2012). It is necessary to evaluate these conditions effectively and to make sense of the conditions under which patients are able to improve and manage the disease to impact patients in a comprehensive manner (Woltmann et.al, 2012). The removal of barriers in implementing the chronic care model requires an understanding of the needs of the patient population and how these strategies have worked successfully in other nations (Garcia-Goni, Hernandez-Quevedo, Nuno-Solinis, & Paolucci, 2012).

This project has been challenging because it is often difficult to obtain adequate support for patients with HIV/AIDS, even in today’s society; therefore, ethical barriers are often present that influence healthcare services for patients with HIV/AIDS. Nonetheless, the importance of the project cannot be disputed and requires full support and guidance in order to ensure that the project is completed in a timely manner. I have sought to communicate with individuals who have provided much-needed input regarding the project and some insight into the HIV/AIDS community, and this has been very beneficial in bridging some of the gaps in knowledge and in supporting the objectives of the project and its longer-term benefits for this patient population.
