Request for Information (RFI) Template A request for information (RFI) is a formal document for requesting information needed to perform an equitable and simultaneous comparison of vendors. Following is a template you can use as a starting point:

Project Title

Statement of Need
The project will support the case management needs to homeless persons between the ages of 25 and 64. There is a significant need for these services because the population does not have access to services to improve their lives and to transition them out of homelessness. It is necessary to develop effective reporting tools to support these services and to obtain proper reimbursements. In addition, adequate documentation is necessary to support the system and to achieve results.

Background
Cerner is an organization that focuses on the development of new systems-based approaches to support excellence in healthcare delivery. Organizations require systems to manage their comprehensive healthcare services and to manage their documentation, and Cerner is an expert in this area. Cerner is focused on “keeping people well” and to improve lives through their approach to supporting effective healthcare services (Cerner, 2013). The organization has significant experience in this field, with over 10,000 partnerships currently in place throughout the world.

Qualifications
Prospective vendors must be able to accommodate clients of all sizes and specialties and work directly with them to develop systems that will support their patients’ most profound healthcare needs. Vendors must also demonstrate their ability to be effective communicators from design to implementation and beyond, and serve clients in a manner that is consistent with industry standards.

Information Requested
The following information is required during the evaluation phase: 1) Number of years the company has been in existence; 2) Number of clients; 3) Type of software; 4) Training opportunities; 5) Licensing opportunities; 6) Ability to upgrade; and 7) Ongoing technical support availability.

Selection of Expert
The criteria included in the evaluation must include 1) Company history, 2) Number of current clients, 3) Availability of technical support, 4) Cost of services, 5) Length of licensing agreement, and 6) Ability to upgrade.

Time for Response
The program requires approximately one-two years to implement and to begin rollout to clients.

Case Management Services for the Homeless – Pilot Program

Company Background
Cerner is an industry leader in healthcare delivery and has a strong history of excellence in providing service to its vast client base. Through advanced technological solutions, Cerner has proven itself as a knowledge-based expert in expanding healthcare delivery solutions to its customers. Cerner works with its clients to design and implement products that will influence the manner in which healthcare delivery is provided to patients in practice settings throughout the world.

Project Description
The proposed pilot project will roll out a new case management system to homeless clients in Massachusetts to improve their access to healthcare services. This client base possesses special needs that are often ignored due to lack of available resources. This vulnerable population warrants these services and a greater focus on their health and wellbeing, as well as their transition out of homelessness. In order to accomplish the objectives of this project, a technology-based system is required to manage documentation and to organize case management services in a manner that will support effective outcomes for this client base. The system must incorporate data regarding this population, including demographic data and health history. This program will allow the expansion of case management services as necessary to improve outcomes for the homeless and to expand the delivery of services to this population on a consistent basis.

Design Requirements
The proposed pilot project requires a comprehensive database that will allow for data entry and tracking of data in an organized manner based on client and program, along with a means of generating reports regarding the program to measure its success. Some customization of the program may be required to meet the needs of the specific demographic group within Massachusetts.

Technical and Infrastructure Requirements
Database design and implementation are required using a server and backup server in order to ensure that data is not lost or compromised at any point from implementation to production and beyond.

Functional Requirements
The system requires a detailed database that will track demographic data, healthcare codes, available plans, and the ability to track data by city, community, individual, family, and other criteria.

Estimated Project Duration
The pilot program will roll out over a three-year period; however, the system implementation will take place over six months to one year, along with the appropriate system testing processes.

Assumptions and Agreements
Vendors are required to provide specific information to determine if the minimum requirements are met, along with other information as required. All proposals will be reviewed for content, price, and specificity.

Proposals will be reviewed in February 2014 and a list of semi-finalists will be selected at the end of the month. Semi-finalists will be asked for additional information in March 2014 and a final decision will be made at the beginning of April 2014.
