My claim was recently rejected on the ground that I didn’t provide an adequate proof of loss of income. Please kindly refer to the attached tax forms for the year 2009 that show my income, mostly earned through a commercially successful film XXX I produced in 2009. The film earned an impressive $XXX in total revenues and I had been working on the sequel when the unfortunate automobile accident rendered me unable to complete the project. As a result of the accident, I have not only lost an estimated $XXX (conservative estimation) in income the sequel was expected to generate during its theatrical run but also future revenue streams from a potential DVD release and licensing agreements with broadcasting media.

The accident has not only cost me financially but has also taken a huge emotional toll on me. The accident has adversely affected my personal life and it has been a daily struggle to not lose hope and keep moving forward. You demanded a reliable proof of income and now that I have provided it, I hope you will reconsider your decision and process my claim request. I look forward to solving this issue in an amiable manner and believe that you will live up to your reputation and do the right thing.
