Adolescent Health Screening, Essay Example

Pages: 1

Words: 376

Essay

Nursing Diagnoses:

Based on this health history and health screening, identify three nursing diagnoses that would be applicable for this client as well as your rationale for your selection of each nursing diagnosis. Include:

One “actual” nursing diagnosis with rationale for choice of this diagnosis.

For this patient, the likely diagnosis is polycystic ovary syndrome, based upon the symptoms that the patient is experiencing over the past six months, such as irregular menstrual periods, increased acne, oily skin, dandruff, and pelvic pain (MedlinePlus, 2015). These issues have contributed to the patient’s overall challenges with respect to her quality of life in recent months and have been frustrating for the client to some degree because of her high level of activity and her young age.

One wellness nursing diagnosis with rationale for choice of this diagnosis.

From a nursing perspective, there must be level of education and awareness regarding the condition and its overall impact on the client in question, and this is best achieved through an explanation of the condition and how it impacts wellbeing (PCOS Foundation, 2015). In this manner, it is likely that a nursing diagnosis of PCOS will require further examination and focus to promote an understanding of possible treatments and symptoms that impact women with this condition (PCOS Foundation, 2015). This is an important reminder that nurses must communicate the primary issues of PCOS to the patient and recognize how to best overcome the most common symptoms through education and outreach (PCOS Foundation, 2015).

One “risk for” nursing diagnosis based on the health screening with rationale for choice of this diagnosis.

With a diagnosis of PCOS, some patients face a higher risk of endometrial cancer, which includes the production of estrogen, in spite of a lack of progesterone (Womenshealth.gov, 2015). In this context, a thickened endometrium forms, thereby creating a risk of irregular or heavy bleeding and subsequent endometrial hyperplasia and cancer under specific conditions (Womenshealth.gov, 2015). This requires an examination of the condition as early as possible in order to prevent further complications and other issues that could impact patients in different ways and create additional concerns that may be difficult to manage.
