Problem Statement
Mount Pleasant has an increasing population of aged people with obesity problems who need access to affordable, collaborative and quality care. At the moment obesity patients are facing challenges in access to quality care therefore the Mount Pleasant Community Healthcare Clinic will provide quality care that will promote positive health outcomes.
A problem statement is important in a project plan since it provides a definition of the problem at hand. In this case the team involved in a project and the stakeholders have an ideal of what they are to focus their attention to (Parker, 2012). A problem statement also plays a significant role in guiding the decision making process in a project since involved individuals have the knowledge about the problem they are required to solve. An effective problem statement helps to concentrate the efforts and focus of a project team towards coming up with a solution to a problem at hand.
Project Mission Statement
The project is aimed at developing a new primary care facility to meet health needs of Mount Pleasant’s increasing aged population. The primary care facility seeks to deliver more reachable, cost effective, collaborative and quality healthcare that will promote patient positive health outcomes. However, the facility will focus more on coming up with a model of care to manage the increasing obesity problem in Mount Pleasant.
A project mission statement is important since it helps to ensure that each person is oriented towards achieving the project’s goals. This is promoted by the fact that it highlights the main purpose of establishing a project (Conder, 2019). A mission statement also helps in informing involved individuals the desired level of performance expected from an established project.

The main goal of developing the primary care facility in Mount Pleasant is to ensure that it will be ready enough to support various health needs among the population. Another goal for the project is improving lasting care model and patient care. Generally, the primary care facility is aimed at providing easily accessible and quality healthcare.

In this case one can compare a project’s progress with its goals to identify whether they are likely to be achieved. The measurable objective for the primary healthcare facility is being located close to public transport and be designed to accommodate the elderly and people with disabilities. An achievable objective for the primary care facility is utilizing teams to consider client’s medical and psychosocial experiences which will ensure that patients are engaged appropriately in a care plan. This will play a significant role in strengthening relationships, promote disease prevention, positive health outcomes and patient enablement. A realistic objective for the projected primary healthcare facility is being is incorporating a pharmacy, radiology, laboratory, nursing services and allied healthcare. The time-limited objective for this project is to be finished and deliver easily accessible and cost effective quality care by 2022.

Resources in this case include funds, people, time and information that will help to achieve the desired goals for the project. The sustainability strategy for this project is identifying the mission and visions that will guide it. The next step will be to identify the issues that matter to the project. In this case the issue is coming up with a primary care facility that will enhance access to cost effective and quality medical care for obese patients. The sustainability strategy will also require the identification of priorities. The priority in this case is developing the primary care facility. The strategy will also require the identification of targets and KPIs. Targets in this case refers to the project’s goals while KPIs are the key performance indicators. These will help to monitor the project’s progress. The strategy that will be able to address political and stakeholder issues is ensuring that all standards are met and having a defined implementation plan that will help to eliminate conflicts in the process. The ethical strategy for this project is coming up with a code of conduct and policies. This will outline the acceptable and unacceptable conduct from all individuals involved in the project. The strategy will help to ensure that the primary care facility will have adequate care professionals with the skills and knowledge to provide quality care to the patient.

Planning the strategies of a project allows managers to make informed decisions when allocating resources. Strategies help to ensure that more resources are allocated to the most important areas of a project (Aramyan, 2016). Strategies also help to identify what a project requires to be complete and allocates financial and human resources appropriately to avoid any challenges in the implementation process. One of the deliverables for this project is having a primary care facility with the ability to support future health needs held by the population. Completion of the project will see the assigning of patients to regular healthcare teams led by a family physician. The regular health care teams will incorporate family physicians, care managers, care coordinators, nurses and other healthcare professionals. In this case the project will ensure that the Mount Pleasant’s population has access to quality and cost effective healthcare. The project will also use the teams to assess a patient’s medical and psychosocial circumstances to include patients in their care plans. This will help to build stronger relationships between patients and care professionals. Completion of the project will promote provision of patient centered care. The structure is necessary in a project plan since it provides a visual of the project’s scope.
