Described symptoms and events that led to his hospitalization. Presented to vomiting, chest pressure, and ED with SOB. The Blood glucose was 569 mg/dL and BUN was 22. Patient was admitted with exacerbation of unmanaged diabetes mellitus, gastritis, and diabetic ketoacidosis.
Medical History and Significant Secondary Medical Diagnosis:
Medical HX: hypercholesterolemia, hyperlipidemia
Medications:
Insulin Glargine 40 units blood glucose management taken 1x per day.
Recommended Diet: Strict 1800 Calorie Diabetic Regime (Fruits, Water, Vegetables, Whole Grains, Low Fat Dairy, Fish, Lean Meats and Poultry)
Learning Need/Goals:
The patient has newly been diagnosed with Diabetes, he is at High Risk for Ineffective Therapeutic Regime Management. The learning needs of the patient are focused on managing glucose levels and preventing any complications to the diabetes. It is necessary to prioritize the needs in order to be focused on the most essential methods in improving his overall health status. Managing glucose levels and diabetes involves: monitoring food, blood and glucose levels, exercise, meal planning, taking medications, skin care, and knowledge of medication and monitoring device. His culture, socioeconomic status, and age as an older, single, African American that has enjoyed a diet high in fats and sugars, as this is common for most people in the southern states. He is not fully aware the dedication to requiring lifestyle changes, and will need to comprehend little changes that will help him return to a normal life.
Expected Nursing Care:
Nursing care and instructions are needed in ensuring that the patient’s pains is low, and is able to return to a normal life through exercise and dieting.Provide importance in understanding hyperglycemic and hypo episodes, and how his race is a contributing factor.
Teaching Plan Objectives:
The patient has a comprehensive understanding of his diagnosis, and why his racial background plays a significant role in Diabetes. Diabetes tends to run in the family for most African Americans, and offspring are more likely to develop diabetes. From the learning objectives, the patient will be able to understand and describe diabetic medication, as well as proper handling and taking. The patient will be able to self-monitor blood glucose levels using blood glucose meter shown through the demonstration from the nurse practitioner or nurse.
The patient will know how to properly care for feet and skin. 4. The patient will be able to explain the benefits of dieting and regular exercising that can improve the blood glucose levels.
Teaching Plan:
The teaching plan is aimed at providing the patient with informative and critical information to make educated and vital lifestyle choices to promote a stable blood sugar and health. For each patient, there must be a comprehensive approach that will clearly provide instruction and information to make lifestyle changes. This includes: education for diabetes and nutrition therapy; food/meal planning; cooperative approach to long-term and short-term goes for changes to one’s lifestyle. This teaching plan stresses the significance to complying with the prescribed discharge instructions and after care treatment. 1. Nurse will provide the patient with teaching materials and teaching times that will occur so that the patient is able to notify friends or family that would like to be present. 2. Nurse will provide discharge instructions for the patient to follow at home, that lists S/S of hypoglycemia and hyperglycemia information that the patient might be confused on. 3. Nurse will show proper handling of the monitoring device to obtain glucose blood sugar.4. Nurse will demonstrate how to draw insulin and administer to different sites.
Nurse will provide instructions on proper foot and skin care.6. Nurse will provide information on the importance of regular exercise on the body and the blood sugar. 7. Nurse will provide the patient with information if blood sugar is 250-300 mg/dL, below 70 mg/dL, or if ketonuria is present for over 24 hours. The patient feels shaky, shortness of breath, fever, blisters on skin, swelling, and carry fast acting sugar. 8. Nurse will provide the patient information when to call he shows symptoms of stroke/cerebrovascular incident, and other diseases. 8. Nurse will provide extra time for the patient to ask any questions, and why his race is a factor.
Teaching Plan Evaluation:
This teaching plan will be effective for the patient, and any patient regardless of cultural background because they are able to follow the comprehensive plan of understanding the disease, learning to manage and monitor glucose sugar levels, regular exercise and dieting, and foot and skin care. The patient is provided with information on a diabetic diet, patient follow up, and informative education to make critical lifestyle changes on their own resonance. This plan will be able to reach any background because although diabetes is prevalent to minorities especially African American, it isn’tcentered or targeted to one specific race or ethnocentric attitude.
