Abuse Assessment Screen (ASS) refers to a multiple-object method of enquiring about a patient’s past and current physical, sexual, and emotional abuse during and before pregnancy. The questions mainly focus on the frequency and severity of abuse and injury. In the event the nurse realizes there is abuse, he or she must ensure the patient is safe. The nurse must ensure all the items likely to cause harm are removed, and full diagnostic PRN tests conducted (Dutton, 2006). The nurse must document all the details, report to the relevant authorities, and take the patient to a counselor to receive psychiatrist evaluation. Further preparations should be conducted to ensure that, once the patient is discharged, she is subjected to a shelter that looks after abused women. Such a move will ensure she has access to the much needed shelter and peace of mind. If the woman is unwilling to part ways with the abuser, the best way is to equip her with methods of identifying when the abuser is intending to mistreat her.
