As the U.S. healthcare system continues to evolve, it is true that fragmentation is a significant issue that requires further evaluation and focus. It requires a high level understanding of the different elements that require expert leadership and practical guidance to overcome some of these obstacles. This is not an immediate process by any means and requires a high level understanding and acceptance of the limitations that the healthcare system currently faces, whereby it does not provide a true sense of discovery and oversight regarding those who face the greatest amount of need. The fragmentation of the system often ignores those who cannot support themselves, thereby contributing to gaps in care and treatment that impact patient outcomes in many ways. Communication in many areas is limited or absent altogether, and patients experience the brunt of these conditions because they impact their health over time. Therefore, changes must be made that will effectively accommodate patients and provide a basis for examining other needs effectively and without further complications to the system.

It is believed that APNs and DNPs will continue to expand their reach within the system and aim to provide a high level of support and encouragement for patients and clinical practice settings. This requires their utmost attention to breakdowns in communication and workflow that may interfere with quality of care in many communities. As a result, when these issues are paid significant attention, they may contribute to a much larger and more comprehensive approach to patient care and treatment that will accommodate patient needs effectively and without complications. These factors will also encourage the development of new directions in healthcare practice that will further support patient care outcomes and quality of care for patients across the spectrum of need so that they are able to experience improvements in their own health and wellbeing over time.
