Case Background
Michael is a fifteen year-old boy who has reported extreme pain during the past six hours before seeking medical attention from the emergency room. The report from the patient consisted of indications of scrotal pain and nausea accompanied by severe pain to the point that the patient is unable to stand. The pain is described by the patient to started off as somewhat dull towards becoming more evasive and continued to worsen by the minute for the past six hours.
Possible Diagnosis based on Evidence
Considering the area where the pain is more rampant, it is assumed that the area affected involves the genitourinary system. The genitourinary system consists of the reproductive organs and the urinary system (Daftary, et al, 16). They are collectively considered to be within one system because of the proximity of their locations. It is distinctively noted that because of such distance from each other, these parts have relative connection especially when it comes to infection being passed on from one part to another (The Merck Manual, internet). Male individuals often experience genitourinary disorders especially among adolescents.
In relation to the case of Michael, it is assumed that he is experiencing pain due to the onset of orchitis, a disorder that often causes the inflammation of the testes which usually includes swelling (RMC, internet). Orchitis also presents distinct pain caused by frequent infection to the area. It could be considered that the infection has already started long before the experience of feeling severe pain started six hours before the case has been brought for medical attention. Due to this, it is suspected that the severity of the pain indicates the spreading of the infection not only in the scrotum but also on areas near the said organ.
Usually, for young males [in their adolescence stage] such condition occurs due to active mumps (Daftary, et al, 16). Due to the fact that the physical checkup was negative for any particular presentation of other underlying ailments, the orchitis that Michael has could be considered concentrated and could not necessarily affect other organs apart from the areas near the affected scrotum.
Suggested Resolutions and/or Treatment
To relieve the pain, it is suggested that inflammatory drugs be taken. This includes naproxen or ibuprofen (The Merck Manual, internet). Meanwhile to control the viral infection from further spreading into other areas, oral antibiotic such as cefalexin and ciproflaxin are suggested to be prescribed (The Merch Manual, internet). In this case, although it is demanded that when severe cases occur, a stronger choice of opiate be prescribed, Michael’s pain mark need not be treated with such type of medication.
Patient Education
It is important to note that the provision of education for the patient be based on the simplest form of explanation that the patient and his family members would understand. Since it is suspected that there is an infection that occurs in the elevated area of the patient’s scrotum, the patient and his parents should be given a clear understanding on how and why such elevation occurs (Daftary, et al, 16). They should also be given a clear understanding as to what the prescribed medicines are supposed to be for. Considerably, the management of pain could also be helped by applying cold and warm compress every thirty minutes interval if the patient would be comfortable with the said setup (Daftary, et al, 16). The parents should be given guidelines as to how such operation could be applied accordingly to not further increase the spreading of the infection.
