This female patient suffers from numerous clinical conditions including acquired pneumonia, unitary tract infection, Alzheimer’s disease, and hypertension. The patient was born and raised in Czechoslovakia with two siblings;  after graduating high school, the patient owned and operated her own salon.  In 1997, the patient immigrated to the United States and eventually married in 1988.    The patient eventually worked as a hairdresser and raised a daughter.    The patient is able to speak Hungarian, Czech, and English- she largely follows patterns of the Czech culture.  The patient wears glasses, does not wear a hearing aid, and due to concerns over nutrition takes Ensure on a daily basis.

The patient’s preferred living setting is in her own room inside NH/ off-unit.  Overall, the patient has numerous interests such as reading novels, listening to different types of music, interested in Hungarian culture, and likes to listen to various types of music (classic, popular, Hungarian),  watching TV shows, and talking/conversing with other people.  She also general activities such as music, reading, spiritual/religious activities, and talking/conversing.    Regarding the patient’s condition, she has short-term periods of confusion, and maybe at risk due to a lack of communication.  The patient is going through a number of interventions in order to improve her condition: a daily routine and sharp cognition through a predictable schedule; maintain existing language capabilities.  It is also recommended to provide her with greater opportunities to socialize and develop interests to accommodate severe sensory deficits.   The overall goal for  the patient is to help the patient acquire greater autonomy maintain the current status over her stay and assist  her in transitioning into her every day life.  The patient’s family ultimately plans to take her home.
