Patient X has shown signs of their condition worsening after six days in care at a hospital in which Dr. Howard is the CEO and President.  The question of concern is whether or not the patient’s current medication plan has taken full effect or if they should consider implementing the EBM order.  The problem with implementing the new EBM order is that both Ms. Jyoti Nasr, Ph.D. (Chief Nursing Officer) and Dr. Ben Jones  (Chief Medical Informatics Officer (CMIO), have not taken the new EBM order to the MEC (Medical Executive Committee).  Dr. Howard, however, brought up the point of the patient’s safety coming first instead of the MEC and if they needed to, an emergency MEC would be called.

With any plan of care, if the patient’s vital signs are getting worse, the staff should respond according to the seriousness of the problem.  (NHS, 2007)   In this case, the doctors and staff are responding to the problem; however, there are pros and cons to their solution.  The doctors and head nursing officer feel that the new EMB order is appropriate for the Patient-X, but it is not validated through the MEC.  If the hospital CEO decides to use the new EMB, the patient could recover and the hospital would have implemented a new innovative plan of care.  On the other hand, if the hospital implements this plan without properly advising the MEC, they could face liability issues.  The purpose of MEC is to promote quality medical care through the careful review of new research. (Medical Board of California, 2011)  The quality of medical care is what the president of the hospital is concerned about.  He feels that the patient comes first and that it is the hospital’s duty to present the best quality medical care.

The case does not discuss any issues with the diagnosis of the patient.  Since the patient is worsening, the medical staff should take review the records and take any type of test that could help determine the best plan of action.   The medical technology field is always advancing and the doctors and nurses should be aware of everything that is available to them.  It is also important to understand the guidelines and ethical use of medical technologies.  (De Miranda et al., 2005)

In order for the patient to receive best quality and most professional manner from the hospital, the hospital CEO should call the emergency MEC meeting and validate the new EBM. If the patient is exhibiting signs of worsening, it is the hospital’s duty to find the best plan of care that is available to him, whether new research or other older implemented EMB.  In this case, there is well documented research that is associated with Patient X’s condition and should be considered.
