Client Description

Mr. Nose is a 41-year-old veteran serving in the military just about 15 years and is thinking about retiring in the next two years. He explained that there were two times when he was sent to combat, but returned without physical injury. Besides, he has a wife of 13 years and two teen age children.

Subjective complaint

Recently Mr. Nose found that he has been losing interest in his job and family. His wife complains that he is no longer anxious to be at home and seldom calls them as he used to do formerly. The last time he was at home Mrs. Nose reported that he asked for a divorcee. When she asked him his reasons, he simply said that she was an incompetent wife, who was cheating.

Objective Findings

Mr. Nose appeared very anxious and was extremely talkative. His clothes were wrinkled and the shirt seemed mismatched with his pants. He was wearing a blue shirt and yellow knee length pants with brown shoes. Mr. Nose’s speech was partially coherent interspersed with bouts of laughter.

Assessment of Progress

This behavior seemed to have progressed to a worse scenario from his last visit when he seemed much more coherent and relaxed.

Plan for Next Session

Plan for the next session is to have Mr. Nose to be further evaluated for Post-Traumatic Stress Disorder (PTSD) and make arrangements for commencement of treatment interventions

Needs for Supervision

Mr. Nose needs to be supervised for suicide attempts and displaying acts of violence on his family or persons in the immediate environment.
