Before physical restraints are used, it is essential to consider other forms of restraint, such as chemical restraint if it is relevant to therapy. However, if physical restraint must be used, the least-restrictive device should be implemented and consequences of constraint should be considered. Clients in restraints must be observed for pain and distress.

Patients with VRE must be kept in a sterile environment and health care staff must ensure to wash their hands and wear a disposable gown before entering into the patient’s facility. 

Nursing interventions that could prevent falls include indicating that the patient is at hazard of falling on his or her room, ensuring his or her gown is an appropriate length, that the IV poles are steady, that adaptive aids work, that grabrails are in the bathroom, and that there are no spills.
