Where did the title, “Chasing Zero” come from?   Explain how the documentary got its name.

Coming from a 100,000 statistical data pertaining to people dying from healthcare harm, TMIT establishes a campaign to lower the said rate to zero anomalies causing the lives of zero individuals who are admitted in hospitals and other healthcare facilities. Relatively, the raise towards a zero-rate of individuals dying from hospital harm could only be attested through improvement of operation and the establishment of good command on the directive medications that they are providing the patients with. The documentary carries on the name of the same campaign as it intends to feature the experiences of the patients who have actually undergone the conditions of healthcare harm and how the health representatives actually intend to respond to the matter accordingly.

Reflect on the following comment made by Braxton Rel’s (hockey player) father when he was unable to get information about possible causes of Braxton’s death.  “…without communication, it makes us feel like Braxton didn’t mean anything to anyone but us.”  Do you think the hospital staff intended to cause the father to feel this way?  Was their silence because they did not care or do you think there were other reasons?  If you believe there were other reasons, what might they have been?

Not necessarily, most of the time, healthcare providers often remain silent to avoid providing unnecessary information that may further put their name into a jeopardizing position. The silence of the staff who attended to Braxton Rel specifically shows how the said members of the organization actually intend to keep their name intact amidst the controversy that is involved in the case of Rel.

Explain the concept of “share rounds” (for change of shift report). What were some of the positive things that staff, patient, and family said about this method?  In what ways would this method contribute to improved patient safety?

Share rounds specifically point out how the transition of information should be handled effectively between staff members of the organization who are taking shifts and are notably taking turn in attending to the same patients for a particular span of time. The interviewees react positively on the said practice as they have observed how such effective transition of work load not only extends the competency of performance of the healthcare providers but also keeps the medication and recovery of the patients to follow a more efficient and more accurate process.
