Sentinel event 40 is Behaviors that undermine a culture of safety, specifically Intimidating and disruptive behaviors. Majority of the health care providers especially the physicians tend to stick the highest possible standards of their behavior professionalism. However, some never consider this as important and are documented to display intimidating as well as disruptive behavior. This was the motivating factor for the Joint commission to take keen interests in behaviors that are associated with undermining the culture of safety in an organization. Consequently, the Sentinel Event Alert (SEA) # 40 was issued to facilitate for the development of standards of leadership that compels the hospitals to adopt an effective code of ethics and a process that facilitate management of inappropriate as well as disruptive behaviors. Reliable information has pointed out existence of behaviors that are directly linked to the occurrence of medical errors (Hickson, 2000).

Addressing such issues has prompted the introduction of Sentinel Event Alert (SEA) # 40 that is intended to address issues of safety culture, enhancement of processes along with leadership in health care settings and its successfully execution has been linked to a drastic reduction in the number and frequency of complaints not to mention the achievements that are attained in augmentation of the safety of the patient (Raines,  2013). Achievement of this initiative is made practically possible through a constructive approach in enhancing care delivery for the patient, efforts of intervention and offering best possible services and prevention of repeated occurrences, with particular attention to understanding potential causes thus leading to transformation in the systems as well as processes that will enhance reduction of potential occurrences in the future, improving the skills on sentinel events, the root causes of such problems and strategies of prevention and sustenance.
