Case Background

A ten-year old girl was brought to the hospital by her mother due to complaints of left ear pain and the occurrence of long-time fever, which was specifically mentioned by the patient to have developed the condition of hearing loss. Based on the mother’s report, the patient has developed a ‘cold’ during the past two days. The fever later on developed with 38. 5oC of temperature. There was also a report regarding the occurrence of vomiting after the last meal taken by the patient.

The behavior of the patient further implicated on how she was actually feeling. She was rather irritated hence resulting to the tests harder to accomplish during the assessment process. Nevertheless, the tests revealed that the patient’s symptoms did not include any occurrence of dizziness, ataxia, ear discharge and tinnitus.

Symptoms Presented

The three primary symptoms that the patient presented which the diagnosis is specifically focused upon is the occurrence of the fever, the pain in the left ear and the possible condition of developing common cold during the past two days which may have enraged the underlying situation in the patient. One specific result being given attention to is that of the occurrence of hearing loss on the patient which needs to be cured as immediate as possible.

During the tests, it was found out that the patient’s tympanic membrane was bulging and was bright red because of the inflammation. CBC And WBC count may help in defining what particular infection is the patient suffering from hence defining whether or not it should undergo further tests and assessment procedures for assurance before the application of treatment. If  particular developments on the physical response of the body towards the infection is desired then CT scan and MRI imaging procedures could e considered. These imaging procedures could provide a better indication in relation to the suspected occurrence of intracranial complications. This however would not be necessary if the problem is resolved early on.

Assessment of Condition

There are five specific conditions that the patient has been tested for. These conditions are as follows:

Otitis media

This condition implicates the inflammation of the middle ear. Characterized by the occurrence of ear pain, hearing loss and fever, this particular situation specifically develops because of the inflammation of the said area.

Tympanosclerosis

The calcification of tissues in the middle ear results to the presence of chalky white patches in the eardrum. Due to the occurrence of these particular change in the ear drum, hearing loss often becomes a developing result.

Otitis Externa

Usually located on the outer section of the ear towards the ear canal, this condition is characterized with pain especially when the said part of the ear is pushed gently. This particular situation often occurs among swimmers who are in direct contact with water pressure at a regular basis.

Bullous Myringitis

This problem occurs due to the inflammation of the middle ear which later on results to the existence of oozing blisters in the ear drum.

Carumen Impaction

This condition is noted through the buildup of earwax which is stored in the ear canal. Usually characterized by itching, otalgia and tinnitus, this particular condition often is irritating if it does not become a source of panic for the patient suffering from it.

Final Diagnosis

Based on the assessment of the actual situation of the child which includes prolonged fever, hearing loss and pain in the left ear, the diagnosis reveals that the patient is suffering from otitis media. Given that the tympanic membrane was bright red and was bulging, it gave an image that the middle ear is inflamed therefore causing both fever and hearing loss. Children are more prone to this condition especially that they are more prone to developing simple cold. It has been noted through the case that the child developed a cold first before the pain in the ear started. Due to such respiratory tract infection, the ear becomes compromised hence is affected by the situation resulting to otitis media [in consideration with this case].  The treatment for the situation shall include antibiotics or the infection and fever medicines to ease down the high temperature of the patient. Once the infection is cleared, it is expected that the hearing capacity of the patient would be back to normal. The patient shall be observed closely within the next 72 hours of medication. After at least four more days, the patient is required to come back for a re-check schedule to make sure that the infection has cleared and the hearing capacity of the child is back to its normal condition.
