The Chronic Care Model with respect to Chronic Obstructive Pulmonary Disease is an important facilitator in demonstrating the value of DNP education at the curricular level (AACN, 2006). This reflects a need to further examine the tools and resources in evidence-based practice to ensure that the appropriate techniques and interventions are used in the Chronic Care Model as appropriate to improve quality of care for the designated patient population. The DNP role is complex and multifaceted, requiring an understanding of collaboration and engagement in order to create an environment where change and progress are possible within the context of patient care and treatment (Montgomery & Bryne, 2011). The DNP-level nurse must also demonstrate leadership skills and capabilities that will have a lasting impact on outcomes and represent a means of advancing the practice environment through knowledge development, strength, mentorship, and education (Waite & Nardi, 2011). For DNP-prepared nurses, moving forward with a collaborative approach and a strong leadership acumen in place will support and engage nurses in providing expert knowledge and resources to improve quality of care for patients at the highest possible level.

There is much to learn within the DNP role regarding how to develop a project and promote change within the context of patient care; therefore, this requires advanced level competencies and engagement that will have lasting benefits for the nursing profession and for the patients who receive care. At the same time, an integrative approach must be considered that will demonstrate the importance of expanding core competencies and collaborating with other disciplines in order to create an environment of change and progress within the context of nursing care and treatment (Hamric, Hanson, Tracy, & O’Grady, 2013). This perspective will improve upon existing knowledge and allocate resources appropriately to ensure that patient care is optimized at all times and supported by a strong leadership core and knowledge base.
