In my opinion it is not appropriate to say that the core of hospital quality relies on an organization, JCAHO that the hospitals themselves control.Importantly, in 2001 the Joint Commission mandated four core measurements through which hospitals will be classified. These included acute myocardial infarction (AMI) and heart failure (HI) interventions. At the same time Joint Commission collaborated with Medicare & Medicaid Services (CMS) regarding AMI, and HF polices common to both organizations with an effort towards alignment(Joint Commission, 2014).
The 7th Scope of Work measures emerged, which formed the basis for hospitals being accredited. From this single example of Joint Commission’s infiltration into hospitals’ specific functionsis indicat6ive that hospitals have no or limited control over what core measures they adapt. In compliance hospitals became implementing measures on 1 July, 2002. No where it was recorded that Joint Commission or Medicare/Medicaid consulted with hospitals specifically during the process. Even if this was dome there is no evidence that hospitals have control over decisions made by these two organizations (Joint Commission, 2014).
However, while mention was made that during the early months of 1999, Joint Commission requested involvement of stakeholders such as health care provider organizations; clinical professionals, consumers of health care, state hospital associations, health care consumers, an in-depth report as to these stakeholders realistic imputs were publish. The policy whereby hospitals will be penalized for violations is indicative that, perhaps, all their concerns were not addressed effectively at the forum convened by the Cardiovascular Conditions Clinical Advisory Panel. Even though the focus pertained to designing core measures for hospital the outcome came from persons considered experts in that particular field. Hospital administrators are not considered experts. Ultimately, they have no control over the measures through which accreditation is obtained (The Joint Commission (2014)
Can an organization that relies on its membership forfinancial viability be truly effective in policing its membership?
Yes, it is my perspective that it can and must offer policies for because it relies on this criterion for viability. Organizations are entities that have collective goals and are linked to their external environment (Handy, 2006). The external environment to a great extent offers an image of the organization through interaction over time. Precisely, this external environment could consist of its members as well as stakeholders. To be truly effective in policing its membership the organization must design strategies whereby its members project relevant goals and mission.
For example, in health care practice a hospital is considered an organization. Members are mainly staff and people who work in the organization. They form the external environment by interacting withpateints as communication of theorganization’s policy regarding quality patient care is articulated. An organization could effectively policy membership through its employment strategies. These techniques must be designed to ensure only skills that enhance performance enter and are maintained because its viability depends on these skills(Handy, 2006).
Poor performance through its members will affect credibility and profits. Therefore, through profound human resource management an organization has opportunities to efficiently policy its membership. Importantly people selected to lead the organizations as members must be screened as a policy measure since they will be responsible for the polices through which further members are recruited(Hewlett, 2006).
