Utilizing a team based medical care solution or a medical home, helps to provide continuity and increased communication regarding patient history and current status of health. The overall objective is to provide the patient with the best opportunity to successfully overcome their health issues and increase the patient’s quality of life (Backer 2007). A medical home provides a comprehensive health care model that enables the health care provided use the advantage of coordinated resources and utilization.

The coordinative efforts of technology and health care provides lessens the amount of administrative roadblocks and enables the healthcare provides to have a holistic view of the patient and their healthcare history. This also provides a personal impact with the provider allowing in depth knowledge and immediate feedback regarding decisions made about the patient’s health (Starfield and Shi 2004). The patient, physician and information are all integrated which provides the patient with the ability to be treated as a person as opposed number. The orientation of the person to the physician builds a relationship that facilitates the coordination of efforts to maximize the opportunity for recovery. Centralized integration and the access to the tools and information will allow the physicians to do the best work of their capability.
