The United States Census has established the American FactFinder website in order to obtain population statistics and information regarding different communities. In Bayside, NY, the population as of 2010 was 28,606 residents, of whom 54.4 percent are White, 35.4 percent are Asian, and 3.9 percent are Black (US Census Bureau, 2014). In addition, 47.8 percent of residents are male and 52.2 percent are female, with a median age of 40.9 years (US Census Bureau, 2014). Within this population, 86.5 percent have at least a high school diploma, and 56.9 percent of the population between the ages of 25 and 34 years have a Bachelor’s degree or higher (US Census Bureau, 2014). There are 11,195 housing units in the community, with 5,934 homes owned and 4,711 rented by local residents (US Census Bureau, 2014). The median household income in this community is $72,837, with a 9.2 percent unemployment rate; furthermore, 1,516 individuals live below the poverty line and for those working, the most common occupations are in the management, science, and business fields (US Census Bureau, 2014). Within this population, 11,444 residents were born in a foreign country, and 52.9 percent of residents speak English at home (US Census Bureau, 2014). This website provides a number of important facts and statistics regarding the population of Bayside, NY in a number of important areas. This community appears to be relatively diverse, thereby creating a unique environment for future growth and change. Since much of the population is educated, there is a greater opportunity to understand how the community might change in the future as new trends emerge and impact the population in different ways. This will ensure that the community is prepared to manage the challenges of its people and their needs as issues arise that require the attention of community members.

This article addresses the significance of different factors in managing chronic illness in the community setting. The authors convey the importance of expanding self-management techniques in order to enable patients to better care for themselves when they face one or more chronic illnesses (Schulman-Green et.al, 2012). Self-management requires effective training and guidance from nurses and other healthcare providers as a means of improving quality of life in patients (Schulman-Green et.al, 2012). It is important to identify the resources that are necessary to accomplish these objectives and to provide patients with the insight that is necessary to reduce the symptoms related to chronic illness (Schulman-Green et.al, 2012). Furthermore, the article considers different monitoring techniques that are available to support patients and to provide a framework for improving their lives in the home environment (Schulman-Green et.al, 2012). The article also considers some of the definitions that are relevant in addressing chronic illness that consider its emotional context and its influence on the lives of patients (Schulman-Green et.al, 2012). Every chronic illness is different; however, each illness requires nurses to understand the condition in order to be able to provide adequate self-care training and guidance to patients (Schulman-Green et.al, 2012). This is an important step towards the discovery of new techniques and strategies to support successful self-management processes that will have a positive impact on patient care over time (Schulman-Green et.al, 2012). Nurses should be effectively prepared to manage patients who face chronic illness and to address the challenges associated with adequate attention to the illness in order to minimize the risks associated with potential outcomes that might be observed in some patients (Schulman-Green et.al, 2012).

This article considers the importance of psychological needs and wants in the context of chronic illness and personal identity. From this perspective, patients with chronic illness are often likely to integrate these experiences into their own identities and cultures (Karnilowicz, 2011). In this context, their disease is a critical component of their core identity, and this plays a role in determining whether or not wellbeing is compromised (Karnilowicz, 2011). These issues are relevant because they describe the importance of recognizing a chronic illness as an unfortunate part of the life span, but to not let it take over a patient’s psyche and emotional state (Karnilowicz, 2011). Similarly, this illness may play a role in current cultural customs and the development of behaviors to accommodate the illness (Karnilowicz, 2011). However, achieving greater quality of life must remain a significant priority for patients across all cultures, even when chronic illness currently exists (Karnilowicz, 2011). The article also notes that the diagnosis of a chronic illness is a life-changing event for many people, thereby impacting their whole health and wellbeing (Karnilowicz, 2011). The ability to effectively manage a chronic illness requires nurses to effectively communicate with patients and to take the steps that are required to facilitate a positive patient identity (Karnilowicz, 2011). However, when adequate measures are not taken, it is possible that a patient might not receive adequate care and treatment for a chronic illness (Karnilowicz, 2011). This warrants an understanding of the issues that arise when patient care is limited, cultural differences get in the way, or other factors contribute to the limitations that patients might experience (Karnilowicz, 2011). These issues are considerable and support desirable outcomes for patients and for others within the field.
