Evaluation Plan: Evidence-Based Practice Proposal, Capstone Project Example

Develop an evaluation plan to be included in your final evidence-based practice project. Provide the following criteria in the evaluation, making sure it is comprehensive and concise:
1) Describe the rationale for the methods used in collecting the outcome data.

Methods used in final data collection entails the use of questionnaires and interviews. Both the patients and the other human resource personnel are given questionnaires to fill in the required information regarding their opinions and views over the treatment of congestive heart failure (Leandro, 2009, p.120). Each of these two groups of persons are given different questionnaires to enable them provide the ultimate solution.

The rationale for using these data collection methods includes the following:

• The responses from the prospects are gathered in a more standardized way. This makes them more objective hence yielding the required results.
• The use of questionnaire and interviews are quick and the required data will be got within the shortest time possible. This will allow for adequate time for the data analysis and processing. However, in certain situations, they may take time to analyze and apply, especially when there is a deficit in resources used in the project.
• The use of the above methods allows for quick collection of data from a large group within a short time. This also helps in improving the research since the questionnaires are got in right time.

2) Describe the ways in which the outcome measures evaluate the extent to which the project objectives are achieved.

The outcome of this project targets to equip the patient with knowledge on the benefits accrued to attending their outpatient classes as well as taking their beta-blocker medication. Some of the anticipated outcome is that the patients learn and understand the importance of taking their medication as per prescriptions, know the importance of attending their clinic classes as well as living a healthy lifestyle that will make them less vulnerable to congestive heart failure attacks (Hamer, 2012, p.391). The patients will also learn various ways of monitoring their weight to evaluate their recovery process.

As a result, the hospitals and health facilities will not be flooded with people suffering from a recurrence of congestive heart failure attacks. Health professionals will also have enough time to attend to other patients rather than concentrating on people suffering from a particular sickness. Consequently, the knowledge earned by the patients during the project run time will be of great benefit to the community around. This is because they will spread the knowledge to their fellow cohorts and develop a common understanding.

3) Describe how the outcomes will be measured and evaluated based on the evidence. Address validity, reliability, and applicability.

The outcome of this project can be measured in terms of its validity, reliability and the level of applicability. In regards to validity, this project is likely to create a great impact that will spread among people in the community. The patients taught will share what they learnt with their family and friends; the same will apply to the friends and as more people get and apply this knowledge, the greater its validity. It is thus not a onetime impact project, rather it is meant to last for the longest time.

The project is similarly of great reliability. Both the patients and the health institution stand a chance of benefiting. Patients will know the correct measures to take when treating congestive heart failure. Also, they will understand the need for following accurately the beta-blocker medicine prescription for better results. This will in turn lower the number of patients suffering from CHF hence creating space in the health facilities (Gazes & Assey, 2010, p.230). The medical practitioners and health officers will also have sufficient time to attend to other patients.

In terms of applicability, this project is universal and applies to all medical institutions dealing with congestive heart failures and the use of beta-blocker medicines. Any health institution can adopt it to help manage the number of complaints in regard to this sickness.

4) Describe strategies to take if outcomes do not provide positive results.

When the outcome achieved do not match the desired results, it is advisable to review the entire procedure. The review should involve assessing any possibility for the deviation in the expected results. When performing the evaluation, aspects such as the resources and the environment need to be placed into consideration. In addition, because the project involves the community, you need to affirm that none of the procedures used contradicts their cultural and religious beliefs.

Consequently, the project can be repeated just to ensure certainty of the achieved results.

5) Describe implications for practice and future research.

The success of this project will help enlighten the community on various means through which they can deal with congestive heart failure and use properly the beta-blocker medications (Dunn, 2011, p.94). This is about the nature of food, medication, and exercise to help them get the best treatments. This will help decongest the hospitals or health facilities that are filled with people having problems of this nature. Further research should focus on inventing new methods and means of dealing with this disease. In addition, the research should seek to create more awareness about the importance of attending health clinics and correct use of CHF medication.
