Introduction
Surveys are methods of data collection obtaining information on a particular subject. The topic for retrieving opinions pertain to ‘everything you ever wanted to know, but was afraid to ask your doctor or primary care provider.’ There are three types of surveys namely case study, sampled and census (Hampton & Vilela, 2012). This would be a sampled survey retrieved through random selection of patients attended by a primary care physician or doctor during the time this survey is active. The instrument for conducting data in the survey would be questionnaires. The time frame is three months.
Primary care physicians or doctors within a selected geographic location will administer these questionnaires to their patients during visit. These questionnaires will be left and retrieved as the program continues. It is important to get feedback regarding transparency of questions to avoid any misinterpretations in completing them. As such, a pre-test of, items will be conducted one month before to assess any discrepancies in interpretation or whether the question is measuring variables inculcated in the research topic (Martin, Childs, DeMaio, Hill, Reiser, Gerber, Styles & Dillman, 2010).
