Primary examples include the Transitional Care Model (TCM), Care Transitions (CTI) Program, Project BOOST, and INTERACT. Each model is reflective of a different approach to optimize the transition of care for older adults when health status has been modified. In addition, concepts such as partnering and health literacy are addressed, whereby nurses and patients partner to achieve the desired transitional outcomes, and providing information to patients and their families to improve health literacy for these individuals. It is important to recognize the significance of geriatric nurses in improving the scope of nursing practice for older adults, as more adults are living longer lives and face important physical and psychological challenges during this stage of life. This article supports these objectives so that transitional care for older adults is optimized at all levels so that patient wellbeing is improved over time. |

It is important for nurses to provide effective guidance regarding discharge instructions prior to the discharge event. It is believed that health literacy for many patients and their families is very limited in many cases because length of stays are shorter and discharges are completed more quickly. As a result, discharge instructions are not always clearly understood and are poorly communicated. The level of health literacy for many patients and their families is thus very low and must be improved so that the quality of life post-discharge is not minimized in any way. From an evidence-based perspective, “teach-back” is comprised of a series of steps which are designed to allow nurses to demonstrate their knowledge to other nurses and for patients to demonstrate their knowledge to nurses so that the subject matter is well understood. This practice enables nurses to achieve improved educational benefits and to be knowledgeable in all areas of discharge planning and education so that patients achieve optimal wellbeing in the home environment. |
