Essay

“Evidence based practice is about formulating a built question, identifying the articles and evidence to answer the questions, critically appraising the evidence and its validity, applying the evidence and re-evaluating the evidence for areas of improvement.” (“Evidence Based Practice: An Interpersonal Tutorial”)With diabetes, it is important to run the relevant blood test to diagnose the disorder. Also, it is equally as important to determine if the physical signs and symptoms exist. Doctors and nurses may want to compare and contrast the existence of another disease based on the patient’s family history. The validity of the diagnosis will be based on the formal patient history and the blood tests and insulin levels. The evidence will be applied according to the blood tests and family history of diabetes including the intermediate family levels.

Filtered and unfiltered sources will be used to make a diagnosis of diabetes. Such sources will include OT Seeker, National Clearinghouse, Physiotherapy Evidence Database, MD Consult among others. Unfiltered sources include PubMed, Ovid Medline, Biosis and others.

The peer reviewed article I choose is Efficacy and Safety of Incretin Therapy in Type 2 Diabetes.  Random trials were used for the purpose of this study. Data sources used were MEDLINE and Cochrane Central Register. Websites were used as un-filtered sources. Random samples were used for 12 week periods compared with placebo and other diabetes medication. Data was extracted by two reviewers independently and differences were resolved by consensus. Fifty-one articles of approximately 355 were retrieved for detailed analysis. Of those only 29 met the standard deviation criteria for the study purposes. DPP4 inhibitors were weighted as neutral. Glugagonlike peptide 1 analogues had more gastrointestional side effects for nausea and vomiting. Dipeptidyl peptidase 4 inhibitors showed increased risk of infections.

Incretin therapy “offers an alternative to hypoglocemic agents currently available for non-pregnant adults with type 2 diabetes.” (Amori, R., Lau, J., Pittas, G. ,2007).  There is a favorable weight change in some patients administered the drug.
