In all health care organization, there is a continuous and increasing need for collecting, evaluating and reporting on data that relates to the safety of the patients, quality measurements as well as initiatives of patient satisfaction (Schutt, 2006). Various tools are used to measure and report data in this regard.

Health System Measurement Project is a crucial tool to track trends in healthcare with regard to care, affordability as well as cost of prevention of diseases as well as technology in health information. The tools used to measure and report data are intended to ensure that all people are able to access data as a crucial step in the openness and transparency of health care system. The users of the tools must access data as quickly as possible on the health metrics of the population from a variety of sources and conduct comparison of trends at regional, state and national levels (Corti & Bishop, 2005).

Another crucial tool that is used to measure and report data is the Kaiser Family Foundation, KFF. The KFF is a good source of the state-level, the MCO-level as well as the parent firm-level data that is related to all-inclusive Medicaid risk-based MCOs.

The National Association of County & City Health Officials, NACCHO is another crucial tool that aids the officials in public heath in understanding, engagement as well as participation in the programs of Health Information Exchange, HIE. Another important that is used to measure and report data is the Agency for Healthcare Research & Quality, AHRQ. This tool makes use of evidence-based system of teamwork that leads to the optimization of patient outcomes through enhancement of communication as well as skills of teamwork among the professionals in health care.
