Dive Heart Foundation, Business Proposal Example

The information in this proposal is for use by Dive Heart Foundation, and shall not be reproduced, disclosed or adapted in its entirety or in portions for any function other than to assess the proposal, provided by the contract.  This does not restrict the use of other information therein obtained from other sources.

Introduction

Dive Heart Foundation has requested Parker Associates Consulting Services to develop job descriptions that will provide guidance to Dive Heart Foundation on job positions for a volunteer coordinator, an office manager, different types of volunteers, and interns with an aim of streamlining the process and functions of Dive Heart Foundation. This request is in response to Dive Heart Foundations performance analysis report on the efficiency of the foundation. This phase of the project aims to remove operational obstacles and assist in the utilization of available resources to maximize outcome.

Statement of Work

This Statement of Work defines the scope of work agreed to in the terms and conditions of the Agreement between Dive Heart Foundation and Parker Associates Consulting Services. This Work Statement and any other relevant attachments represent the complete Agreement between the two concerning the work and it overrides any previous written or oral communications.

Alterations to this Statement of Work done in harmony with the procedures provided within this document and may necessitate adjustments to the expected schedule, charges, and/or other provisions of this Agreement.

Scope of project

The objective of this task is to provide a guideline for the streamlining of the operations within Dive Heart Foundation to ensure effective performance. A summary of the major goals of the proposed project is to:

• Define and establish the infrastructure available for the operations of the foundation and the expected purposes and desired outcomes of this infrastructure
• Design the roles of the various job positions in the company. Provide a clear perception of the functions of the employees involved in the operation of the foundations duties as well as propose additional job positions.
• Research the best practices available to the owner of the foundation in order to come up with a framework for the smooth operational functionality
• Create job descriptions that provide the desired qualifications for the positions the expectations of the positions and the desired functions of the position provided in the running of the foundations operations. This proposal has limitations  to solving issues with infrastructure, task Sharing and role clarity
• Provide operational guidelines for operations in regard to Dive Heart Foundation

Key Assumptions

This Work Statement and Parker Associates Consulting Services estimate to perform the Statement of Work founded on the following basic assumptions. Any effect resultant from changes to these assumptions may result in alterations to the Charges, Estimated Schedule, and/or other provisions of this Agreement.

1. Dive Heart Foundation will provide information of most recent employees, the available infrastructure and the roles of employees for analysis by Parker Associates Consulting Services.
2. All publication costs for the production of the proposed guidelines covered by the Agreement will be for Dive Heart Foundation expense, and the same is separate from the statement of work and not included therein.
3. Parker Associates Consulting Services will make available a copy of the proposal and the guidelines covered by this Agreement for Dive Heart Foundation.
4. Dive Heart Foundation may duplicate as many copies of the guidelines as desired but for internal use only.
5. Dive Heart Foundation may not reproduce copies of the proposed guidelines for use outside of Dive Heart Foundation or for sale to other parties.
6. Parker Associates Consulting Services gets priority concerning any further, and subsequent changes desired because of the initial phase and reserves the opportunity to provide training and development related to the provided guidelines.

Dive Heart Foundation Responsibilities

The tasks expected to be performed by Dive Heart Foundation listed below and performed by Dive Heart Foundation personnel.

• Provide a copy of Dive Heart Foundation most recent employee positions qualifications and roles.
• Production and duplication of final guidelines for distribution to Dive Heart Foundation people-managers.
• Respond to queries regarding project in an appropriate manner.
• Provide changes to Statement of Work with the clear understanding that these changes may affect the Charges, Estimated Schedule, and/or result in modifications of other terms of this Agreement.
• Offer any information that does alter the scope of this Work Statement and do so in a timely manner.

Parker Associates Consulting Services Responsibilities

The responsibilities defined in this segment are, in addition to the tasks stated, in this Agreement and the objects listed within the Assumptions, herein, and are to be delivered to Dive Heart Foundation at no extra cost. Parker Associates Consulting Services performance bases its  perfomance upon the subsequent fulfillment of these responsibilities:

Parker Associates Consulting Services will…

• Contact Dive Heart Foundation with any queries relating to the project in order to make clear the final expectations concerning the guidelines.
• Generate from its existing knowledge of infrastructure role Clarity and task Sharing to facilitate research into these topics for the purposes of satisfying the Statement of Work.
• Answer promptly to queries relating to the project progress, and to provide regular statements of progress as per Dive Heart Foundation request.
• Provide final guidelines in a format and manner in which it will be sincere and cost effective for Dive Heart Foundation to produce, duplicate and distribute the guidelines.

Estimated Schedule

Parker Associates Consulting Services shall deliver the work products described in this document, in accordance to the following schedule:

• Final guidelines delivered and ready for publication to Dive Heart Foundation before the lapse of 60 days from the date of signing the Agreement.
• The initial set of guidelines delivered to Dive Heart Foundation for review before the lapse of 45 days from the date of signing the Agreement.

Completion Criteria

Parker Associates Consulting Services will fulfill its obligations provided for by this statement of Work when any of the subsequent events occurs:

1. Parker Associates Consulting Services finalizes the work described in section 2.8.
2. Parker Associates Consulting Services reaches the end date of 60 days from the execution of this Statement of Work.
3. Parker Associates Consulting Services or Dive Heart Foundation terminates the project in accordance with the provisions of this Agreement.

Estimated Charges

Parker Associates Consulting Services will provide up to the estimated 100 hours of services for this Statement of Work at an hourly rate of $20 per hour. The total estimated funding for this Statement of Work is a maximum of $2000. Dive Heart Foundation paid for the fixed amount in two installments, one at the halfway through the project that is 30 days after the date of signing this Statement of Work, and the other installment upon completion and delivery of the completed guidelines.

Deliverables

Parker Associates Consulting Services will deliver a guideline on the use of infrastructure, role Clarity, task sharing, and job descriptions for the personnel at Dive Heart Foundation concerning the Statement of Work.

Additional Terms and Conditions

Should Dive Heart Foundation and Parker Associates Consulting Services determine that interviews with the employees, volunteers and interns are a value-added process for this project, these interviews billed at $50.00 per hour and may be conducted on Dive Heart Foundation premises through the appropriate and efficient methods. Dive Heart Foundation will supply the details including the communication information for the appropriate individuals for the interviews. Any such proposed interviews and the compilation of data collected from the interviews will result in an extension of the contract by 10 days beyond the stipulated date as stipulated in section 2.5 therein.
