The article is well written and pertinent as well, though the article was lacking in the full exploration of all the available options to patients. They did however explore the main ones and a valiant effort indeed. Not only law but also education have limited nurse practitioners as well which was pointed out to in the article. An addition would be to change legislation in New Brunswick and the rest of Canada to allow nurses to specialize in limited aspects of cardiology.

The lack of physicians has indeed created a service gap. The article offered an interesting solution to patience with recent heart failure, which was to include phone based service. It is my humble proposition to include recent advancements in telemedicine into the mix to provide a more holistic solution not only to patience but also to nurses as well. The will serve the purpose of solving patient accessibility as well.

The paper does keep to topic very well however I would contend that there is an element of flow that I found unsatisfactory. The definition for instance of the heart failure the different treatment options fell rather late and in my observation should have introduced earlier in the article. The definitions were well articulated with a clear rational keeping with the abstract.

The matching of statistical evidence to back up the general claims as well as the specific ones drove the point home several times and consistently appeared throughout the article. This was especially evident when dealing with potentially controversial elements like allowing patience to have an alternative care provider in case of mild cases by noting the staff limitations in terms of numbers to patient ratios.
