The article gives an overview of how the quality of health care system has improved in recent years, and examples illustrate aspects like diverse settings, demographic groups, and conditions (p. 845). The article also looks into offering insights into nationwide quality by looking at existing categories that encompass preventive, acute, and chronic care types. The article gives definitions on common terms relating to quality of care given to patients in the United States. The deficiencies in care in this article are fueled by public incidents, and the growing evidences has increased in the United Kingdom and the United States. The number of initiatives launched by each of these countries is mentioned in this article, but without explicit consideration of the multilevel change approach, nothing is achieved. The approaches include group/team, system level/environments, individual, and organization. At all levels, the article emphasizes that attention be given to culture, information technology, leadership, and team development. Tradeoffs between centralization and decentralization must have a balance of the contingent factors evident in both countries in order to sustain the impetus for improvement of quality over time. As a result, a multilevel framework of change and its associated properties is provided because it assesses the progress.

In such instances, the health care reform systems helps give focus on issues that improve on the health and care value for their patients. The system referred in this article is physician led because it revolves around the aspects that physicians can contribute in bringing change to the health care system (p. 1108). The article looks at three goals and or principles that guide the physician, which includes patients goal; organization of medical practices around medical conditions and care cycles; and results in terms of adjusted risk and measured costs. The principles discussed in length and gives readers an overview of the contributions of physicians in bringing change to the health care system (p. 1110). The evidence scan is an article that looks into two questions: 1) What training types concerning formal improvement techniques of quality are available for health professionals? 2) What evidence is present on the most effective methods for training clinicians concerning quality improvement? (p. 2). The article gives a message to the readers on what that article aims to achieve and thus, gives focus to the importance of having quality improved in the health care system. It is evident that the quality of health care systems round the world needs to be improved and maintained because it relates to the health of the population. As such, training of medical practitioners and professionals needs implementation because it leads to improvement of knowledge, attitudes, and skills (p. 4). The scope, training examples, and the most effective approaches of training are discussed to give an objective view to readers on how training influences the quality of health care delivered (pp. 10-33). The article is relevant because it gives informed evidence on techniques applied in learning institutions and hospitals or health care systems; on how to uphold and improve their excellence of service delivered.

Quality in the health care system started back in the 20th Century as is evident in this article. The article looks into bringing the changes bound to take place in the health system, in the coming century, to give medical professionals the chance to embrace the advancements to be experienced. The message of this article is directed towards the private and public sectors of the health system and to the federal, state, and local governments. The message of the article looks at all aspects that affect people from all backgrounds in the United States is knowledge for policy makers, managers, taxpayers among others. The article gives a reflection of the changes bound to take place in the 21st century and thus, is educative because people will be able to embrace the changes early in time.
